Mental health team


In our surgery the mental health team assesses patients who are struggling with any aspect of their emotional/mental health. They will be offered an assessment with the mental health nurse who will be able to provide support or signpost to the most appropriate service/intervention.

If you feel that you would like to speak to the mental health nurse either for yourself or for a person in your family, please submit an online consultation request via your practice website or contact your GP surgery reception to ask for an appointment, stating the reason why you would like to be seen and the best telephone number to call you on and an appointment will be arranged.

In a mental health emergency, please contact the all-age crisis team on 0800 0328 728. This is for adults and young people and is staffed 24/7.

Patients can also access the All-Age Wellbeing Portal self-referral system, this is for adults and people with learning disabilities. The portal enables patients with the opportunity to refer themselves using the new online referral form. It also allows carers and professionals to refer on behalf of the patient, where consent has been provided.
